Please complete this secure and confidential registration form for Group Therapy at Counsel RVA.

Each session is a closed group. This means the spot is held only for those who are registered. Once the group begins, no other participants will be allowed to join the group but will need to register for the next group. Participants are expected to attend all sessions.
*Payment will be pulled from the card on file the morning of each group. Those who pre-pay will have the pre-paid amount pulled the morning of the first group meeting. Participants understand that when attending a closed group they will be billed for any session not attended as the spot was held specifically for that member.
